Currently a primary care nurse at the San Vicente 1 Healthcare Centre (Alicante Health Department-Dr Balmis General University Hospital), where she works as a community case manager. Case management services are targeted at patients (and their carers) of any age with complex or advanced chronic disease requiring high intensity care. The aim is to ensure the continuity of the care and transitional care programmes, facilitating the coordination and integration of multiple professionals and services.
